Federal Nursing Student Loan MPN

Before Wright State University can begin crediting your Federal Nursing Student Loan funds to your student account you must first review, sign, and return this Federal Nursing Loan Master Promissory Note (PDF) to Enrollment Services. The MPN contains information specific to your loan. The MPN is the legal binding document that you must sign when you get a federal student loan. It lists the terms and conditions under which you agree to repay the loan and explains your rights and responsibilities.

Important Information About This Document

• Do not leave items blank. Be sure to complete Section A, numbers 1-5.
• Print clearly in black or blue ink. An illegible document will be returned to you. No electronic signatures will be accepted.
• Complete and return the original Master Promissory Note (MPN) to Enrollment Services in person or via mail. Do not send a copy or facsimile. An original signature is required.
• It is highly recommended you retain a copy of this document for your records.
