Minor Power of Attorney Instructions

Being a minor without a parent or legal guardian in the United States is a major challenge, as minors will find it difficult to obtain health care, counseling, medication, housing, and other critical services without a parent or legal guardian signing documents for the minor student. You may obtain documents establishing a person in the United States as: (1) your legal guardian; or (2) your parents' or guardian's attorney-in-fact. 

Instructions for the Minor Power of Attorney form (PDF) are below. Make sure all information is filled out for the child, the parent(s)/guardian(s), and the appointed attorney-in-fact.

  • The parent(s)/guardian(s) must initial in the area next to said box.
  • The beginning and end date need to be filled in. The beginning date should correspond with the date on which the POA is signed; the end date should correspond with the child's birthday.
  • Governing Law. "Ohio" should be in the blank; unfortunately, it has to be filled in by hand or prior to printing.
  • Parent/Guardian Signatures. Names must be printed; dates must be clear.
  • Acceptance. Again, name must be printed; dates must be clear.
  • Witnesses. Two witnesses are needed for this section. Names, addresses, and dates must be clear. Signatures are necessary.
  • Notary Acknowledgment. Remember that the notary must be attesting to the parent(s)/guardian(s) signatures.
  • The completed form must be in English.
