Eligible Dependents and Verification

Eligible Dependents

Employees appointed to work at or above a 75% full-time equivalency basis are eligible to enroll their dependents in select benefits with Wright State University. Your enrolled dependents must meet the following eligibility definitions, and you will need to provide dependent verification as noted below:

Dependent Type

Eligibility Criteria

Child (up to age 26)

Biological child, stepchild, legally adopted child, or a child under permanent legal custody/guardianship, or covered by a Qualified Medical Child Support Order.

Disabled Dependent (26+)

Unmarried child certified as disabled and financially dependent on the employee.

Spouse

An individual married to a benefit-eligible employee and supported by an active government-issued marriage certificate.


Dependent Verification

If you are enrolling your spouse and/or dependents in one or more of the following benefits, you will be required to provide supporting documentation:

  • Medical
  • Dental
  • Vision
  • Tuition Remission
  • Dependent (Spouse/Child) Life Insurance

Supporting documentation for the enrolled dependent relationship will be required at the time of hire, qualifying events, or during Open Enrollment. Periodic audits may also be conducted.

 

Required Documentation for Dependent Verification

Dependent Type

Required Documents

Biological or Adopted Child (up to age 26)

  • Birth certificate or court document (paternity test, divorce decree, or Qualified Medical Child Support Order)

Stepchild (up to age 26)

  • Birth certificate or court document (paternity test or divorce decree) and confirm eligibility of the spouse (as noted in spouse requirements above)

Legal Guardianship Custody (up to age 26)

  • Court document assigning a minor child to an employee under permanent legal guardianship or legal custody

Foster Child (up to age 26)

  • Agency assignment or court document grating child to an employee under foster care

Disabled Dependent (26+)

Spouse

  • A government-issued marriage certificate, and
  • A joint document (Ex: utility bill, mortgage statement, etc., dated within the last 90 days).

 

Spousal Insurance Coverage Policy

If you are enrolling a spouse on your medical coverage, you are required to adhere to the Spousal Insurance Coverage Policy and complete the Spousal Employment Verification Form to determine your spouse’s eligibility for primary or secondary medical coverage.
