Below you can find all of the forms you need to request service or support for technology at Wright State. If you can't find the form you're looking for, contact the CaTS Help Desk for further assistance.

Account Requests
- Account Application
Request or renew a computer account for non-paid faculty/staff, student employee delegated accounts, and long-term guest accounts.
- Account Application Form (Boonshoft School of Medicine)
This form is for BSOM faculty and staff computer accounts only.
- Account Name/Email Change
Request to change campus username account and/or email address. Note: You must first have your information updated with HR (faculty/staff) or the Registrar (students).
- Account Verification Form
Request access to your personal account information if the Help Desk is unable to verify you and you cannot appear in person.
- Alumni Account Application/Renewal
For Current Wright State users (including Alumni that have an active Alumni account)
- Alumni Account Request (PDF)
For former Wright State graduates requesting a new account that are unable to login to our systems
- Alumni Account - Request to Close
As a graduate of Wright State with access to your Alumni email account, fill out a request to have your account closed.
- Guest Account Request
Short term guest accounts for visitors to Wright State needing internet access sponsored by a department. This form requires authentication by the department sponsor.
- Office 365 Online Community Request
Communities include SharePoint Sites, Microsoft Teams, and Groups, all of which give users a place to collaborate with co-workers, classmates, and others. This form will give you more details about which Office 365 community best fits your needs, including the ability to set up a shared email/mailing list accessible by different members of your department.
- Retiree Account Request
Extend your account after you retire (must be an employee of Wright State for 10+ years)
- STEM School Student Computer Account Application (PDF)
As a Dayton Regional STEM School Student, submit this form for a Wright State University computer account.
- Workshop Account Requests (PDF)
Accounts for Non-WSU individuals attending a Wright State sponsored workshop in a CaTS computer lab.
Access Requests
- Banner Admin Access
Fill out a request to access various restricted modules in Banner (Student Info, Financial Data, etc). Applicants must have their campus username and password to request access, and will need authorization from their Department Manager and the departmental approver for data being accessed.
- Cognos Access Request Form
This application is for Wright State University staff and faculty with an educational interest and need to access information from one or more of the module areas listed. Applicants will need authorization from their Dept. Chair, College Dean, or Dept. Director.
- Cognos Project Request Form
Use this form to submit a work/data request to the Office of Institutional Research & Effectiveness.
- Network Shared Area Request
Used to grant access to a department shared area or restricted area.
- Non-Student Invoicing Application Access Form
Use this form to request Non-Student Invoicing Access.
- Responsibility and Confidentiality Form (PDF)
This form confirms that a user will treat all information available to them with their account access as highly confidential and understands the possible legal implications if they do not treat that information as such.
- SCT Workflow Request
This form should be used to request access to the Banner Workflow system for Grade Change and/or Major/Minor workflows.
- Server Registration
Register a new or existing server on the WSU network.
- uAchieve Access (Faculty and Staff Only)
Submit a request for access to uAchieve as a faculty or staff member.
- WINGS Express Faculty and Advisor Access
Request for individuals needing access to student information.
- WINGS Express Finance and WrightBuy Access (fillable PDF)
Submit a request for access to either the Finance and/or WrightBuy modules through WINGS Express.
Service Requests
- Support and Project Request (formerly system modification request)
Submit support and project requests to Information Services here. This includes issues in production (live), reporting a bug, new feature/project/report requests(requires authorization with campus username and password).
- VTS Services Request
Request video services through the Video Technology Services department.
Classroom Support
- Classroom Equipment Instruction Request Form
Request individual classroom equipment instruction.
- Classroom Software Request
Request to have special software installed in a classroom.
- Report a Classroom Problem
Report any problems in electronic or computer classrooms.
Network Access
- Campus Housing Network Jack Activation
Request to have the network jack activated in your dorm room (current Wright State students only).
- Network Drive Increase Request
Request a size increase of your network drive.
- VPN Registration/VPN Request Form
Request access to the Wright State Virtual Private Network (VPN).
Miscellaneous
- Appearance Release (PDF)
Grant Wright State permission to use your name and/or one or more portraits, pictures, photographs, video and audio recordings of you ("Appearances"), or reproductions or derivatives of the same, in any form of education, communication, and/or promotion purposes, unless otherwise noted.
- Create a New Pilot Course (for faculty)
Submit your course information and we'll create a new course for you in Pilot, Wright State's learning management system.
- Duo Security Token Purchase Request
Use this form to request to purchase a Duo Security Token for use with Wright State's Two Factor Authentication System.
- Exception Request
Request an exception from a current CaTS policy in order to accommodate a technological need or request that falls outside of the policies set forth by CaTS.
- Non-Supported Product Form (PDF)
If you prefer to purchase a product not supported by CaTS, fill out this form acknowledging your understanding of the responsibilities that come along with using this non-supported product.
- Pilot Course Cross-Listing Request (for faculty)
Submit information for a course you would like to have cross-listed in Pilot, Wright State's learning management system. Cross-listing means that students in more than one Wright State class will have access to the same online class.
- Unified Messaging
Unified messaging is the ability to have voicemail sent to your email as an audio file. Submit this request if you would like to access your voicemails via your Wright State email, in addition to dialing x1010 on your desk phone.
