About Microsoft Forms

Microsoft Forms is a new part of Microsoft 365 Education that allows you to quickly and easily create custom quizzes, surveys, questionnaires, registrations, and more. This service is accessible to all students, faculty, and staff. 

Note: Microsoft Forms is currently in 'Preview' mode; the look and functionality may change over time.

Benefits and Key Features

When you create a form, you can invite others to respond to it using any web browser or mobile device. As results are submitted, you can use built-in analytics to evaluate responses. Form data can also be easily exported to Excel for additional analysis or grading. 

Microsoft Forms vs Qualtrics

What is it? Forms is an application through Office 365 that allows you to quickly and easily create custom quizzes, surveys, questionnaires, registrations, and more.  Qualtrics is a research software program that allows you to create surveys using sophisticated analytics. 
When to use it
  • When a simple survey is needed quickly
  • Registration tool for small events
  • When a simple quiz is needed quickly
  • When surveying a smaller audience
  • When collecting and filtering large amounts of data
  • When specific rules need to be used as a user takes the survey
  • When surveying a medium to large audience
How to access it
  1. Login to WINGS with your campus 'w' username and password
  2. Click on the Office 365/Email icon
  3. Click the gear icon on the top left corner of the landing page
  4. Select 'Forms'
  1. Visit wright.qualtrics.com
  2. Sign in with your campus 'w' username and password

 

Getting Started

You can access Forms using the following methods:

WINGS Website

Log in to WINGS with your "w" username and password

Click on the Office 365/Email link in the top right corner to access your Microsoft 365 account

Click on the grid in the top left corner of the Microsoft 365 screen

Select the Forms application

Log in to Office 365 (http://outlook.wright.edu) with your Microsoft 365 username and password

Usage Instructions

To begin using Forms, sign in to your Wright State Microsoft 365 account and open the Forms application in the menu bar.

Note: If you do not see the Forms icon in the application grid, click the 'View all my apps' link at the bottom.

The Forms icon will look like the image below before it is set up in your account:

Click the tile to begin set up; you will not notice any changes as the application is setting up. Once set up is complete, the Forms icon will appear like the image below:
